Dear Mighty Hawk Band Families,

Hello everyone! We are REALLY looking forward to closing out the month of June with a strong Monday Full Band Rehearsal and a great week at the Summer Percussion Camp. We hope to see you there! Important Update regarding physicals below...

We have just received new guidance from UIL and the district that requires ALLstudents to have a current Pre-Participation Physical Examination (PPE) on file, as well as a completed medical history. This is different than what was advertised at our previous full band meeting, and we sincerely apologize for the inconvenience. Guidance regarding next school year from our governing bodies has been very slow to roll out and mostly unadvertised. Due to this recent change in policy, please have these forms into the band staff as quickly as possible. To be more specific on who must complete the forms...

  • All current 9th Graders (Class of 2025)

  • New PPE and Medical History required

  • All current 10th Graders (Class of 2024)

  • New PPE and Medical History required, UNLESS an examination by a doctor was completed for the 2020-2021 school year.

  • All current 11th Graders (Class of 2023)

  • New PPE & Medical History required

  • All current 12th Graders (Class of 2022)

  • New PPE and Medical History required, UNLESS an examination by a doctor was completed for the 2020-2021 school year.

I have attached a copy of the documents that must be completed to this email for your convenience. Please be sure that BOTH the Medical History (1st page) and the Physical Examination (2nd page) are completed. If you have already turned in the Medical History page, you may just turn in the Physical Examination page once completed by a medical professional. We must ask that you please have these forms completed and on file with us as quickly as possible, and we sincerely apologize for this inconvenience.   • 5:00-7:00pm - Full Band Inside Rehearsal

  • 7:00-8:00pm - Full Band Outside Rehearsal
